Add refractivity to 5-layer atmosphere

The new 5-layer atmosphere constructors merged in !359 (merged) don't currently setup the refractivity.

We should support enabling the refractivity in this constructor as well for various models.

To upload designs, you'll need to enable LFS and have an admin enable hashed storage. More information
